Fatal carbon monoxide incident caused by boiler failure, investigation ongoing

A carbon monoxide leak that killed an 11-year-old boy in December was caused by a boiler failure, according to an update from Regina Fire on Friday.

Through a scene examination, Regina Fire and Protective Services determined that the incident was unintentional and was caused by a boiler that experienced “significant internal failure,” which resulted in a substantial release of carbon monoxide.

Regina Fire has seized the boiler and plans to have it undergo a technical examination to determine the exact cause of the failure.

Although the scene examination is done, Regina Fire says the investigation is ongoing with the Regina Police Service to determine if there is a criminal aspect to the incident.

Henry Losco, 11, died following a carbon monoxide leak in his family’s apartment building, located on the 1800 block of Albert Street on Dec. 19, 2025.

The family says they appreciate the work of investigators.

“I think it’s important that they go to the root of what happened to this boiler, right, and we appreciate that they are giving updates,” Sergio Losco, Henry’s father, said.

Sergio Losco was found unconscious in the fourth floor apartment. He was taken to hospital and survived. He says CO detectors did not go off in the apartment. The family is waiting for further answers.

“The police investigation is ongoing and TSASK (Technical Safety Authority of Saskatchewan) is doing their investigation too and those are ongoing so nothing can be disclosed at this time,” he said.

The family continues to grieve the loss of their son.

“Since losing Henry, I spend a lot of time, there’s a lot of guilt even going on. What should I have done differently? How could I have kept my family safe,” Marina Hills, Henry’s mother, said.

She said there are important questions that tenants must ask their landlords.

“What is it that’s heating them? Are things up to code? Are they being maintained? Do the carbon monoxide detectors work? When was the last time that they were checked,” she said.

Regina Fire says it will continue to investigate what happened in the building. Additional findings are expected to be revealed in the future.
